Council Recognition Request Form

There are several life events and contributions that are worthy of recognition. At the Town of Ajax you can acknowledge these achievements and bring awareness to specific causes. Requests should be submitted a minimum of four weeks in advance of the date the recognition is required. 

Please note, you can only submit one event per form.

Legislative and Information Services staff will contact the applicant within a week of receipt of the recognition request to notify them whether the request has been approved or denied.

A limit of one (congratulatory certificate, flag raising, proclamation, special achievements certificate) request for any individual, group, team or organization will be issued within the calendar year.
